An exams leave letter is usually written by employees pursuing their education, in order to seek their employer’s approval to take days off from work to appear in important exams. Since you are going to write to your senior, make sure that your tone is professional and formal.

Proof read your leave letter a couple of times to make sure it is free of typos and grammatical mistakes. Use professional but simple language. Write your leave letter clearly and precisely to avoid any confusion regarding the number of days you want to take off. If, in case, you want to cover up for the work which you have missed out on, make sure that you mention the whole plan for covering your work up. In case you are planning to work from home, you can mention that as well. Write down the contact details where your seniors will be able to contact you easily.
